Protecting your privacy is important

At the Physio Gibson clinic, we are committed to protecting your personal information. Your personal and medical information will not be shared without your consent.

Personal information collected includes name, phone numbers, email address, address, occupation. Medical information includes age, information related to current and past conditions, results of past medical tests, your activities, etc.

The information is provided by the patient verbally or through electronic documents. The information gathered by electronic documents managed and stored by Medexa clinic management software. Medexa Inc. is a Quebec company that stores the data in Quebec.

Information can also be gathered in paper form at the request of the patient. This information is kept in a locked filing cabinet and only accessible by the authorized person.

Information sent by text and email

It is not recommended to share personal information by text or email. If you choose to voluntarily share information through text or email, the information provided is used strictly in the patient’s interest by the clinic and will not be shared.
Communicating with the clinic through the contact link on the website by e-mail, requires you to provide your name, telephone number and e-mail address.
These emails will be delt with the same confidentiality as paper correspondence. Despite this and protecting the information provided with personal passwords, any information sent by over the internet can not be absolutely secure.

Access to your personal information

You have the right to access your personal information and can request modification if the information is inexact. Please see the contact information below.

Retention of information

We retain your personal and medical information for a period of at least 5 years in accordance to the regulations of the Order professionnel de la physiothérapie du Québec. After this time period, the information can be destroyed or deleted ensuring that no unauthorized person can access it.

Withdrawal of consent

Consent to collect and maintain your personal information can be withdrawn at any time. This right, however, may be limited by the regulations of the Ordre professionel de la physiothérapie du Québec.
